We know that Multiattack is an action a monster can take. If a monster has two actions, then it stands to reason it can Multiattack twice if it has two actions, in the same way that the Fighter can Attack action twice with Extra Attacks when using Action Surge.
Can a creature grapple with Multiattack?
Generally, no. The monster’s Multiattack entry will specify not just the number of attacks they can take, but which of their attacks (defined in the same section of their statblock) they are allowed to take. They can’t just swap any of those out with grapple or shove or any other attacks they might have.
Is Action surge an op?
It’s very op as a splash ability on a level x character. Action Surge scales with total character levels, rather than class levels. As a result, it’s going to be a very attractive dip for high-level characters. It’s not going to be very attractive for low or mid level characters as a dip.
