Open up your enchanting screen by right-clicking the enchantment table. The enchanting screen will show, with a place where you can place items, and 3 buttons. Select the tool or armor or other item (like a book) to be enchanted. Hoes, shears, and horse armor cannot be enchanted on a table.

It is possible to obtain enchantments on items without an enchantment table, and this would completely outclass the enchantment table. Using this ‘magic anvil’ along with the regular anvil completely ruins the use of the enchantment table and the game progression.

They are generally using editing tools, like MCEdit, to create the worlds and add into them the items that you are mentioning. Like the Super Hostile maps that you can get (I believe its Enchanted Caves that has a collection of ever more powerfully enchanted signs that are really very handy weapons in that map :))
