Given that, while polymorphed, the wizard is the ape, damage taken by the ape counts under “whenever you take damage”. Thus, the wizard must take a constitution save to maintain concentration (with the ape’s constitution modifier). Being polymorphed does not break concentration.
Do you have to concentrate on polymorph?
Logic 1: Polymorph changes everything about the character, including the mental stats. Your intelligence goes down to an animal’s, so you do not have the wherewithal to even think of concentrating.

If a spell must be maintained with concentration, that fact appears in its Duration entry, and the spell specifies how long you can concentrate on it. You can end concentration at any time (no action required). The rule is very short and only says “at any time (no action required)”.
Can a PC lose concentration by casting polymorph?
As far as the PHB is concerned, a PC can only lose concentration by taking damage, casting another spell, or being incapacitated/killed. Shy of other special rules, there is nothing in there about any other effect like polymorph (which by definition evidently requires concentration to maintain the effect, do not have the book in front of me).
How long is the concentration rule for polymorph?
Polymorph does not state otherwise. The Polymorph spell states a Duration of Concentration, up to 1 hour. Concentration rule states nothing about losing it if losing intelligence (and even Feeblemind states that the creature can identify friends, follow them, and even protect them).
