Optifine is a “client side” mod…it doesn’t get installed on the server, only on the clients that are connecting to the server. However many mods do need to be installed on both the server and client… basically any mod that adds an item to the game (a block, an entity, etc).
Is Optifine needed on server?
OptiFine is a client-side only mod, meaning it only needs to be installed on your computer in order to run. Installing it on a server, for which it was not designed for, is going to cause performance problems, if not outright crashing.

Optifine is safe to use, so it is not the reason as to why you were banned. This sounds like you were hacked, and the person that hacked your account cheated in mini game and got your account banned, unfortunately hypixel has a rule that your account is your responsibility, so you won’t be unbanned.

When do I use graphics mods such as Optifine?
Actually adding in Optifine to a server (with Forge) will throw a ClassNotFound exception. (I did this to my pack a few days ago, wondered why it wouldn’t load!) No Optifine doesn’t need to be patched server side -just on your client. All it does is handle your graphics for the game , shader packs etc and gives you more settings on your client.

No, as long it is not a mod that affects gameplay, only the graphics. It will not affect your world, nor does the server need to be patched. I am not certain about OptiFine as I never tried it, but it should work fine when only modded client-side.
Do you need Optifine for 1.9.4 Minecraft?
So from https //optifine net/downloads, there is a section for Minecraft 1.9.4. But all of them say Forge and a 4 digit number, which confuses me. Does that mean all of these Optifine mods come with forge, (the mod needed to load other mods) or need forge to run?
Where do I put the Optifine file in Minecraft?
Place the new file we just extracted from the installer before, into the mods folder. Open up Minecraft with Forge, and Optifine will be installed and working. You may need to restart Minecraft for the changes to take effect.
