The character was “being flailed about” in the grapple (which also restrains the PC), and used Disonnant Whispers successfully. It moved the frightening thing (PC) away from the Otyugh, which would have otherwise been dragged in the grapple, and since it used it’s reaction, it stayed where it was.

The grappling rules do not say anything about size other than that you cannot grapple something two or more sizes larger than you, and that your speed is halved during a grapple unless the grappled creature is two or more sizes smaller than you.

Why does 5e grant extra movement with dissonant Whispers?
Previously it has bugged me that 5E seemed to grant extra movement as a reaction only to targets of the Dissonant Whispers spell, whereas you normally couldn’t move as a reaction unless you’d Readied a move (which is basically a delayed Dash).

You are only limited if you use a bonus action to cast a spell as well. You can use Dissonant Whispers and then hit them with all the shots of a scorching ray in the same turn if that is what you want to do. War caster is always a cantrip anyway. Incorrect. Any Spell with a casting time of 1 action that only targets the retreating subject is valid.
Can a hydra use dissonant whispers to run away?
Can Dissonant Whispers spam force a hydra to consume its reactions running away, or will the creature not have to spend its reactions on running and remain able to use them for opportunity attacks?
