STEP 1: THE KEYS To begin your journey all you need to do is find and kill a Keywarden. There are four Keywardens and each one can be found in one of the four acts in Diablo III. The Keywardens in the first three acts drop a key, and the Keywarden that appears in Act IV drops the crafting plan for the Infernal Machine.

Can you use three infernal machines at the same time?
To prevent this from happening what you should do is use three Infernal Machines to open all three portals at the same time. Make sure to plan out your game session ahead of time so that you will have enough time to enter all three portals and kill all three boss pairs.
Where to find Infernal Machine of fright Act 4?
Act 4 — Nekarat, the Keywarden of Fright drops the Infernal Machine of Fright and is found in the second tier of the Gardens of Hope. This will reward you with Heart of Fright upon completion of its event. 1.3.
