Once you kill Astrid you get a quest that says to talk to guard, which leads you to the questline to wipe out the rest of the brotherhood. If you choose to kill her, you get one more mission. If you join you get a whole questline of varied quests. I’m a good guy too, but I didn’t want to miss all that.

If you let Cicero live, he will join you once again after the Dark Brotherhood main quest is completed. You can use him as a follower, and he will be available in the new Dark Brotherhood sanctuary located at Dawnstar.

You can get Cicero’s armor even if you WANT to spare him but not after you spared him. The way to get the armor and keep him alive involves a high pickpocketing skill level. It’s only once he starts getting up that you’re able to pickpocket his clothes.

Can you find Cicero after destroying the Dark Brotherhood?
Cicero will disappear after Destroy the Dark Brotherhood! Although Cicero will still be alive, unfortunately you can’t find him when you destroy the dark brotherhood.
Can you get Shadowmere if you kill Astrid?
No. The quest “Destroying the Dark Brotherhood” activates only if you attack Astrid in the Abandoned Hut, before you join the Dark Beotherhood. It is impossible to get Shadowmere or Cicero after that.
