“Melee” attacks can be against any creature within your reach (generally 5 feet), while ranged attacks can be made against any creature within the stated range of the attack. In some cases, an attack form has two ranges; attacks at the longer range are made at a disadvantage. If the wording says “melee weapon attack” you can do an unarmed strike.

You can’t prepare move and attack as a readied action, you have to pick one. Also, you only get one attack even if you would normally get more than that. RAW implies that it can only be an action, so you can’t ready movement. You could, however, ready the ‘Dash’ action, which is pretty close.
When to attack when target is within range?
If you want to attack when the target is within range, don’t make it whenever they attack. GM “which enemy?” GM “which orc?” GM “ok, and which square?” GM “cool, and which attack?” PC “OH COME ON!” I also feel that being too particular sets up the dm for “cheating”.
