If an Unconscious character takes damage while at 0 HP, they automatically fail one death saving throw, or 2 death saves if the damage is from a critical hit. Massive Damage can still outright kill the character so damage should still be rolled and if it equals or exceeds their max HP then they die (PHB pg. 197).
Does poison affect death saves?
On fail, the target is poisoned. Damage from a saving throw isn’t part of the attack that caused the saving throw (for instance, it doesn’t get rerolled on a critical hit). It’s true this can incur a second death or concentration saving throw, but it also means you’re less likely to be killed from massive damage.
Do spells get crit 5e?

When does a creature have disadvantage on an attack roll?
The creature has disadvantage on Attack rolls. An Attack roll against the creature has advantage if the attacker is within 5 feet of the creature. Otherwise, the Attack roll has disadvantage. A restrained creature’s speed becomes 0, and it can’t benefit from any bonus to its speed.
What happens when an unconscious but stabilised character is attacked?
If an unconscious character is attacked, the attacker will have advantage on the attack (if melee) from the Prone condition; if it hits, it will be an automatic critical hit because of the unconscious condition which is 2 death save failures.
